Observation status

What does Observation status mean on a medical bill?

Observation status means the hospital kept you in a bed, ran tests, and watched you overnight, but billed the stay as outpatient care rather than an admission. The room looks identical from where you are lying. The billing does not: outpatient rules apply, your share is figured differently, medications can be handled differently, and for Medicare the status also affects what is covered if you go to a nursing facility afterward. The status is set by a written order, and it sometimes changes during the stay.

What does Observation status usually cost?

Under observation the stay is priced as a series of outpatient charges instead of one admission, so the same nights can add up differently. Which rule set applies depends on the order in the chart, not on how the room looked.

What should I watch out for with Observation status?

  • Ask whether you were admitted or under observation, since the bed looks the same either way
  • Ask for the date and time of the order that set or changed your status
  • Ask about the status before discharge when nursing facility care may follow, because Medicare rules turn on it
