Denial code CO-45

What does Denial code CO-45 mean on a medical bill?

CO-45 is the line where your plan cuts the provider's charge down to the price it will recognize. The provider asked for one number, the plan's contract or price list says a lower one, and the difference comes off the claim instead of moving to you. The CO letters mean the cut comes from the deal the provider signed with the plan, so it is normally not part of your share. Some plans send the same cut with PI, which means the plan trimmed it on its own rather than under a contract. Either way, the number to watch is the one on the bill you receive.

What should I watch out for with Denial code CO-45?

  • Compare the CO-45 amount on the EOB with the balance the provider's bill is asking for, because they should not be the same number
  • Ask the billing office in writing which contract term moved a CO-45 write-off onto your statement
  • Ask your plan whether the provider was in network on that date, since the answer changes what the write-off covers
